Responses to this form are shared with tutors who work with your child. Use it to update us with any medical or contextual information that will help us best support them i.e. neurodiversity diagnoses or testing, family bereavements or separations. Arrival, Departure and Breaks

For health and safety reasons, everyone attending the Saturday Programme must sign in and out using the sign-in sheet provided on site. All Parents/Guardians are responsible to ensure the safe collection and/or transportation of their students to and from the Saturday Programme venue

Year 7 opt-out form

If your child(ren) are in school Year 7+ we do not expect parents to be bring them in for classes, but of course you still can if you wish! However, if you do NOT give permission for your child to arrive or leave on their own either at the start and end of the day or for breaks, you must complete this online form.

Tuck Shop Credit Form

If your child attends the Saturday Programme unsupervised and does not have the means to make contactless or chip and pin purchases at the tuck shop, they can get pre-paid tuck shop credit which we will then monitor and tell you when it is running low. To purchase tuck shop credit please complete this online form.

I Can Sing! Performance Permission form

When preparing for a performance, I Can Sing! students will need to sign a permission form outlining additional rehearsal times, costume requirements, and other relevant information. This form will be emailed across to you to access directly, however it will also be accessible once it is live here.

Medication Declaration Form

If your child will need to take medication whilst with us on a Saturday as a rolling consideration (ie inhalers or epi-pens) or for individual circumstances (ie antibiotics) you will need to declare this to the office. Be aware that tutors cannot administer medication and so if it is needed during your child’s time at SP a parent will need to be on site to administer for students below year 5. Mini 2 Instrument Choice Form

Once Mini 2’s have had tasters on each instrument they will be asked to fill out this Instrument Choice Form. It will be emailed across to you to access directly, however it will also be accessible once it is live here.

I Can Sing! Performance Ticket form

When preparing for a ticketed performance you will be told when tickets are available for purchasing. This form will be emailed across to you to access directly, however it will also be accessible once it is live here.

Year 6 Unsupervised Arrival/Departure Permission Form

If your child(ren) are in school Year 6, and you wish them to arrive/ leave site unattended at the start of end of their day or during breaks, please complete this online form.

Mini 2 Progress Decision Form

During the Summer term Mini 2’s will need to chose which programme they want to do the following academic year using the Progress Decision Form. This form will be emailed across to you to access directly, however it will also be accessible once it is live here.
